POSITION SUMMARY The Home Health Nurse will provide care to patients in their homes under the guidance of a physician.
Responsibilities include performing patient visits and monitoring conditions, conducting tests, and reporting information back to the physician.
The Home Health Nurse will also be responsible for making appropriate adjustments to patients’ in-home management plans for COVID-19 and chronic diseases and encourage medication and lifestyle management adherence.
The Home Health Nurse serves as a liaison between the clinic and the COVID-19 team and provides nursing expertise to the COVID-19 Care Team.
PRIMARY ACCOUNTABILITIES Travel to patients’ homes and community events Utilize team-based care approach to develop and manage care plans in coordination with the clinic team Vaccinate and test for COVID19 in a clinic setting and outside in the community if need it.
Participate in any community outreach done to address impact of COVID19 Actively participate in Genesis Family Health Outreach Community Testing and Vaccination.
Complete blood pressure, glucose, and COVID-19 testing as needed Learn and apply clinic policies and procedures to COVID-19 Care teamwork Monitor for signs of COVID-19 and chronic disease complications Protect patient information and maintain confidentiality according to HIPAA guidelines Listen to and answer questions and concerns of family members and patients, or assist connecting to appropriate professional to answer questions Educate caregivers, family, and patients on ongoing care and lifestyle management options Provide suggestions for improved healthcare options to physicians, family, and patient Monitor patients and compile reports for physicians Attend professional development and trainings, as needed Participate in weekly COVID-19 Care Team huddles Provide education and consultation on COVID-19 and chronic disease management at community events Assist COVID-19 Care Team in selecting accurate and appropriate educational materials for populations experiencing health disparities Provide medical expertise, technical assistance, and coaching to CHWs and Patient Navigator Collect data and/or report on activities that demonstrate progress toward grant objectives to report to Program Director Collaborate with COVID-19 Care Team to develop end-of-year success stories POSITION REQUIREMENTS RN Relevant Kansas license and certification Ability to execute work within parameters set by a workplan Ability to create, implement, and refine clinical workflows using CHWs and Health Navigators Expertise in chronic disease prevention and management Understanding of COVID-19, including prevention, contact tracing, vaccination, and management Awareness of and ability to effect change in health equity issues and approaches to reducing health disparities in at-risk populations Ability to communicate effectively and courteously with patients/families from diverse cultural backgrounds; sensitivity to the often-stressful situations patients/families are facing; effective interpersonal skills Good written and oral communication skills; excellent organizational skills Experience and working knowledge of community resources Ability to work well with diverse members of a multi-disciplinary team, including patients, families, health care providers, community resource providers, co-workers Ability to describe medical information to patients in plain language Oral and written fluency in English Technical skills in Microsoft Office and other programs.
Ability to navigate electronic health records, basic database entry, electronic communication, report documenting, etc.
Ability to fluctuate from working independently and within a team, shifting tasks, prioritize tasks, problem-solve daily, and complete work within deadlines Valid driver’s license and reliable transportation PREFERRED QUALIFICATIONS Oral and written fluency in Spanish, or a language other than English Firsthand lived experience or close connection to the community being served Experience working in community-based or populations-based clinics Prior experience working with public health strategies Additional Requirements Position requires sufficient hearing level to effectively communicate with people directly or by telephone.
Moderate physical activity.
Requires handling average-weight objects up to 15-20 pounds, assisting with patients and standing and/or walking for most of the day.
Work involves considerable exposure to elements such as unpleasant odor and noises.
While performing the duties of this job, the employee is regularly required to maintain sufficient visual acuity to read, write and operate equipment.
