Position Summary: Under the direction of the Director of Nursing, the Nurses Aide/Patient Care Tech facilitates the provision of age-appropriate patient care by performing specific tasks and support functions as delegated by a Registered Nurse (RN)/LPN or Licensed Medical Provider (LMP).
Job Duties: Performs technical duties including transportation of medical supplies, patients or labs to facilitate timely patient delivery and carry out the multidisciplinary care plan for each patient.
Delivers physical care and comfort measures to assigned patients in a safe and efficient manner.
Provides emotional support and considers patient and family needs.
Organizes and prioritizes workload to complete assignments during the shift and response to Stat orders.
Appropriately assists patients at the bedside with their individualized exercise/activity program.
Safely assists and guards patients during ambulation.
Monitors patients safety, compliance and maintains a safe environment for patients and co-workers.
Demonstrates good observation and communication skills to enhance interdisciplinary approach.
Obtains and maintains integrity of specimens in accordance with policies and procedures and the RN/LPN or LMPs orders.
Recognizes changes in patient status and reports appropriate information to the RN, Charge RN and/or LMP and documents these changes according to policies.
Coordinates information flow through the patient care unit to promote effective use of time and resources, prioritizes assignments and requests to ensure optimum patient care and unit performance.
Demonstrates patient advocacy, assuring patients' rights to privacy and confidentiality.
Maintains and role models a professional demeanor in interactions with patients/families and colleagues.
Helps to ensure proper supply levels of materials and equipment within the department and assists with re-stocking the department.
Participates in open communication within nursing unit and other care units and departments to ensure appropriate and accurate patient care.
Attends and participates in the practice of emergency procedures, fire drills, evacuation procedures and internal/external disaster drills.
Participates in patient care through planning, interventions, re-evaluation, written and computer documentation, observation and comparison to established standards.
Monitor patients condition and assess their needs to provide the best possible care.
Observe and interpret patients symptoms and communicate them to nurses/physicians.
Foster a supportive and compassionate environment to care for patients and their families Monitors patients safety, compliance and maintains a safe environment for patients and co-workers.

Minimum Qualifications: Education: High School diploma or equivalent preferred Experience: At least one (1) year of experience in a health care setting Licensure/Certifications: Basic Life Support (BLS) Certification required upon hire, Certified Nurse Assistant (CNA) Certification preferred Physical Requirements: General Activity: Sit: < 2 hrs.
