*GENERAL DESCRIPTION:* Reporting to the Information and Referrals Director, staff provides services to older residents, persons living with disabilities and caregivers.
Duties include providing confidential help with personal problems, information about available services, insurance counseling, referrals and follow-up for needed services, and advocating on behalf of each customer.
The Information and Referral Specialist functions as the key human link between the customer and the service system within the community.
*PRIMARY RESPONSIBILITIES:* Provide information, referrals, and insurance counseling to assist customers with obtaining public and private programs and services for which they are eligible.
Initiate referrals to agency and community service providers and coordinate activities in order to provide an integrated system of service delivery.
Provide support counseling on topics and services affecting older adults and individuals with disabilities.
Develop an effective working relationship with area agencies that provide programs and services to older adults, persons living with disabilities, and caregivers.
Conduct follow-up on services received and provide advocacy.
Promptly return phone calls, emails and messages.
Document service delivery and maintain client records and confidentiality in accordance with SWKAAA Policies & Procedures.
Attend all mandatory training.
Complete a minimum of 15 hours of Information, Referrals and Insurance Counseling Continuing Education each year.
Perform other related job related duties as assigned by supervisor.
*QUALIFICATIONS:* Bilingual fluent in English and Spanish.
Associate's degree in gerontology, human services, psychology or related degree with at least one (1) year experience in the geriatric and disabilities service field.
Strong knowledge base and interest in working with older adults and persons with disabilities.
Ability to work independently and with a team, strong communication skills, organizational skills, and counseling skills.
General knowledge of medical delivery systems, community resources, and public benefits programs.
Ability to establish and maintain appropriate boundaries and confidentiality.
Basic computer skills.
Willing to travel to surrounding counties.
Valid Kansas driver's license and proven driving record.
Successfully pass a background check.
*EMPLOYMENT STATUS:* Non-exempt; full-time; benefit package; vacation and sick accrued time off; some evenings required; some travel required.
Job Type: Full-time Pay: $33,280.
00 - $35,360.
