We are looking for a dedicated professional like you to be a part of our Home Care Services team as a Full-Time RN Case Manager.
*This position is scheduled Monday-Friday, 8:00AM- 5:00PM with an alternating On-Call Schedule.

Our Home Care Services Department at Hemphill County Hospital District is dedicated to providing expert medical assistance to patients while in the comfort and familiarity of their own homes.
A few responsibilities of the RN Case Manager include:
- Providing individualized care based on the patients unique needs.
- Creates a safe and professional environment in the homes of our patients.
- Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation.
- Responsible for directing and coordinating all nursing care for departmental specific patients based on established clinical nursing practice standards.
- Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.
- Supports the organization vision and mission.
- Utilizes knowledge of patient’s age and cultural diversity into the provision of patient care.
- Contributes to the provision of quality nursing care through performance improvement techniques that demonstrate positive outcomes in patient care.
Education/ Certifications/ Licenses Requirements:
Graduate of accredited school of nursing required
Current registered nurse’s license in the State of Texas
Current BLS certification
ACLS or PALS certification within 6 months of hire (as applicable to department)
Willingness to be crossed trained to other areas
Minimum Experience, Abilities Required and Special Requirements:
- Previous experience in Direct Patient Care in a hospital or hospice setting.
- The employee must possess basic working knowledge of computers, and be able to enter necessary data into a computer.
